Product scope
ClearClaim Verify is eligibility workflow software for front-desk, billing, and practice teams before the claim.
The product helps users organize payer-returned eligibility information, review check details, create handoffs, export report PDFs, preserve history, and save follow-up work.
ClearClaim Verify is not medical advice, legal advice, payer-contract advice, a guarantee of reimbursement, a guarantee of coverage, or a final patient-responsibility determination.
Insurance-card extraction is an assisted data-entry feature. Extracted values may be incomplete or incorrect and must be reviewed before an eligibility request is submitted.
Customer responsibilities
Customers are responsible for confirming patient, payer, provider, service, authorization, referral, plan, contract, and organization workflow requirements before quoting, billing, collecting, or making operational decisions.
Customers must use account access, roles, invitations, billing settings, and team workflow features only for authorized organization users.
Customers should not submit PHI through public website forms, demo requests, or public support email unless ClearClaim has provided a secure support channel.
Customers represent that they are authorized to submit patient information and insurance-card images and to request eligibility information for treatment, payment, or healthcare operations.
Customers are responsible for verifying extracted card values and for maintaining accurate provider identity, Tax ID, contact, and payer-enrollment information.
Accounts, usage, and billing
The current usage-pricing version, promotional allowance, graduated rates, billing period, accrued usage, payment status, and amount due are shown in the product or checkout before activation.
The billing terms shown at checkout, in an order, or in a customer agreement control charges, renewals, cancellations, refunds, taxes, and payment timing.
Only authorized account owners may activate usage billing, manage payment and budget controls, change billing, or manage organization-level access.
Production use involving protected health information may require an accepted Business Associate Agreement in addition to these Terms of Use.
Payer enrollment and third-party services
When an authorized customer starts payer enrollment, ClearClaim may transmit provider identity, NPI, Tax ID, contact information, enrollment selections, and required documents to payer-enrollment services and the applicable payer.
NPPES lookup confirms only the public NPI listing and does not establish credentialing, network participation, PECOS eligibility, or payer approval.
Payer requirements, enrollment availability, requested documents, review times, and approval decisions are controlled by the applicable payer or service provider. ClearClaim does not guarantee approval or completion time.
Eligibility transactions, payer directories, enrollment, card extraction, hosting, authentication, billing, email, and other product functions may depend on third-party services. Availability and returned data may be affected by those services.
Customer data and product rights
Customers retain their rights in data they submit. Customers grant ClearClaim the limited authority needed to host, process, transmit, protect, and display that data to provide and support the service.
ClearClaim retains its rights in the product, software, workflows, documentation, branding, and aggregated or de-identified operational information that does not identify a patient or customer.
Customers must not upload content they do not have authority to use or instruct ClearClaim to process data for an unlawful or unauthorized purpose.
Acceptable use
Users may not attempt to bypass authentication, access another organization, misuse API keys, scrape public pages, reverse engineer protected workflows, interfere with service reliability, or submit unlawful content.
Users may not use ClearClaim Verify to make clinical decisions, misrepresent payer-returned data, or treat incomplete eligibility information as guaranteed financial responsibility.
Users may not copy, resell, sublicense, or use the service beyond the access their organization purchased or ClearClaim authorized.
Availability, changes, and suspension
Public sample results, pricing examples, and screenshots are illustrative unless explicitly stated otherwise.
Service availability can be affected by maintenance, payer systems, connectivity, enrollment, payment providers, and other third-party services outside ClearClaim control.
ClearClaim may update product features, usage pricing, public pages, and these terms as the service evolves. The effective date on this page identifies the current version.
Formal customer agreements may include additional or different terms for production use, privacy, security, support, billing, and compliance.
ClearClaim may suspend access needed to protect the service, prevent unauthorized use, address nonpayment, or comply with law or payer requirements. Account termination, data return, and deletion are governed by the applicable customer agreement and retention obligations.
